If your child needs medication during the school day or needs access to emergency medication, please download the attached Medical Authorization Form.  Completed paperwork should be signed and submitted to the office, Attn: School Nurse.   

Please note:  The “Over The Counter” (OTC) Medication Form is available in your Skyward Family Portal and must be completed online.
